Circulation and Potency of Carfentanil
In recent weeks, a new version of fentanyl has been circulating in Canada: carfentanil, a molecule 100 times more potent than fentanyl. It has led to a record number of overdose deaths, since it is virtually impossible to properly dose street drugs containing it.
According to the U.S. Drug Enforcement Administration (DEA), carfentanil is a synthetic opioid approximately 10,000 times more powerful than morphine and 100 times more potent than fentanyl. The presence of carfentanil in the illicit drug supply in the United States is alarming, as its extreme potency could cause a surge in overdoses and overdose-related deaths—even among opioid-tolerant users.
Carfentanil also poses a significant threat to first responders and law enforcement officers who may come into contact with the substance. In any situation where a fentanyl-related substance such as carfentanil may be present, police and emergency services must strictly follow safety protocols to prevent accidental exposure.
Information for Officers and Public Safety
Carfentanil and other fentanyl analogues present a serious risk to public safety, as well as to first responders, medical staff, treatment providers, and laboratory personnel.
These substances may appear in several forms:
- Powder
- Blotter paper
- Tablets
- Transdermal patch
- Aerosol
Some forms can be absorbed through the skin or accidentally inhaled.
Safety Guidelines
- Exercise extreme caution: only properly trained and equipped law enforcement professionals should handle any suspected fentanyl-related substance.
- Watch for signs of exposure: respiratory depression or arrest, drowsiness, disorientation, sedation, pinpoint pupils, clammy skin.
- Seek immediate medical assistance: in any suspected exposure, call emergency medical services right away.
- Be ready to administer naloxone: the antidote for opioid overdoses; multiple doses may be required.
- Remember: carfentanil can resemble cocaine or heroin powder. Do not sample it; secure the substance and follow approved procedures.
Lethality of Carfentanil
Carfentanil is used as a tranquilizer for elephants and other large mammals.
The lethal dose range in humans is unknown. However, carfentanil is about 100 times more potent than fentanyl, which can already be fatal at doses as low as 2 milligrams, depending on the route of administration and other factors.
Situation in Toronto
According to journalist Isaac Adams of Radio-Canada, carfentanil—an opioid up to 100 times stronger than fentanyl—is circulating at unprecedented levels in Toronto’s drug supply, according to a new report from the Toronto Drug Checking Service (TDCS).
The organization detected carfentanil in 14% of 193 fentanyl samples tested in August, the highest level since monitoring began in 2019. This is well above the 1–2% rates observed between April 2023 and this spring.
“It’s terrifying,” said Zoe Dodd, co-organizer of the Toronto Overdose Prevention Society.
“In 2019, 35% of overdose deaths were linked to carfentanil, even though we had more supervised consumption sites. Now we fear people will die preventable deaths.”
Common Symptoms of Carfentanil Overdose
- Severe respiratory depression: very slow or stopped breathing
- Loss of consciousness / extreme drowsiness: difficult to wake
- Extremely constricted pupils (pinpoint pupils)
- Cold, clammy, sometimes bluish skin (cyanosis)
- Wheezing, shortness of breath, low heart rate, low blood pressure
- Nausea, vomiting, disorientation, confusion
These symptoms typically appear within minutes of exposure or ingestion, as carfentanil acts extremely rapidly.
Immediate Actions (Emergency Response)
- Call 911 immediately
- Check breathing; if absent, begin rescue breathing
- Administer naloxone (Narcan) if available; repeat every 2–3 minutes as needed
- Secure the scene: do not touch the substance, keep others away, and inform emergency responders
Important Notes
- Naloxone is safe to administer even if the cause of collapse is unconfirmed.
- Carfentanil is extremely powerful: several naloxone doses may be necessary.
- In cases of inhalation or skin contact: move the victim to fresh air, wash skin with soap and water (avoid alcohol-based cleaners).
